How to Register

Check you live within the boundary area

To access our registration process, first check you are in our catchment area. If you have any questions about your eligibilty join this surgery please contact us.

Registration takes 2 weeks time. You can make a routine appointment with a doctor after this time.

If you live within the boundary area, please complete our new patient registration.

When you register, it's helpful to have your NHS number. You can use the NHS website to find your NHS number.

Temporary Services

If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.

To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.

Disabled patients and residents of care homes may be registered by authorised persons acting on their behalf or by post.

Registration process

It will take up to 2 weeks to complete your registration.

Medications

You have 28 days to collect medication from your previous surgery while we process your registration.

If you are taking regular/repeat medication you will need to be seen by a doctor before issuing. Documentary proof of your medication is essential to provide you with these prescriptions.

Outstanding appointments

Let us know about any pending appointments with your previous surgery, and we can assist you.

Form completion

Please fill out the registration form clearly to avoid any delays.

Contact details

Ensure that your address, contact details, and previous GP address are filled in legibly.

Your Named GP

All our patients at Bower Mount Medical Practice have a Named "Usual GP".

We are asked to ensure that all our patients know the name of their "Named GP".

We would encourage our patients to check with reception or with the doctor, to confirm that they are certain of the identity of their named GP, when they have an appointment at the surgery.
